If you fill out the CHAMP Plan inquiry form on the Site, we collect the information you choose to provide — typically your name, business name, email address, phone number, and any notes you add to the form. We only collect what the form asks for. We do not sell or rent this information.

CHAMP Plan inquiry form submissions are used to follow up with you about the CHAMP Plan and to connect you with the CHAMP Plan product team at Champion Health (see the Disclosure Policy for details on our relationship with Champion Health).

Automatic log information is used for Site security, performance, and general analytics.

We do not use your information for advertising, profiling, or automated decision-making.

Who We Share Your Information With

If you submit the CHAMP Plan inquiry form, we share the information you provided with the CHAMP Plan team at Champion Health so they can respond to your inquiry. This sharing is the entire point of the form — by submitting it, you're asking us to forward your request for more information.

We do not sell your personal information. We do not share it with advertisers, data brokers, or anyone else outside the specific purpose above, unless we are required to do so by law.

Where Your Information Goes

CHAMP Plan inquiry form submissions are stored in a customer relationship management (CRM) system we maintain and are then shared with Champion Health through the CHAMP Plan website. Once the data is in Champion Health's system, their privacy practices apply to that copy of the data. We encourage you to review Champion Health's own privacy policy if you want to understand how they handle it on their end.

How Long We Keep It

We retain CHAMP Plan inquiry submissions for as long as is reasonably needed to follow up on the inquiry and maintain records of our business communications. If you want us to delete your information, contact us at the address below and we'll do so within a reasonable time, subject to any legal record-keeping obligations.

Your Choices

You can always choose not to fill out the CHAMP Plan inquiry form. If you've already submitted it and change your mind, email us at info@gopowerwellness.com and we'll remove your information from our system.

Children

The Site is not directed at children under 13, and we don't knowingly collect personal information from children. If you believe a child has submitted information to us, contact us and we'll delete it.

Security

We take reasonable steps to protect the information you give us. That said, no website or internet transmission is 100% secure, and we can't guarantee absolute security. If you're concerned, you can always contact us by phone or mail instead of using the form.
